The purpose of this database is for you to be able to review all the survey records for the employers who submitted surveys within your jurisdiction. Each jurisdictional representative will be issued a unique username and password that will allow you to access the surveys for the employers you worked with within your jurisdiction. You will not be able to access other jurisdictions’ surveys or employee surveys for employers that other representatives worked with.
